General FAQs
Rescheduling Policy
We understand that unanticipated events occasionally happen in life. In our desire to be effective and fair to all of our patients and out of consideration for our providers’ time and resources, advanced notice of 24 business hours is required when rescheduling an appointment.
If you are unable to give us 24 business hours notice, the following missed appointment fees will be charged: $80 for a chiropractic appointment, $80 for a massage appointment and $95 for an acupuncture treatment. Thank you for your understanding and consideration.
Parking
The entrance to our parking lot is on SW Salmon Street adjacent to the corner of SW 16th Avenue.
There are three parking spaces in the lot marked EQPDX. They are numbers 4, 11, & 13. If those three spaces are occupied, there is metered street parking available nearby.
New Patient Paperwork
If you are scheduled for a new patient appointment, you have received an email with our new patient intake and consent forms. Please complete and submit the forms online before your scheduled appointment, or come to your appointment 15 minutes early to fill out paperwork in the lobby.
Privacy
We take privacy very seriously and have a copy of our most updated HIPAA Privacy Policy posted here, in our clinic lobby and available for you to view whenever. We do our best to follow the HIPAA Privacy and Security Rules and invite you to learn more on the US Department of Health and Human Services Site.
Chiropractic Care

Massage Therapy
Massage is the act of working on the body with different amounts of pressure to relieve pain and stress. Techniques include using hands, elbows, forearms, and fingers to manipulate muscles and tissue to release toxins and increase blood flow. Read more about the different techniques here.

Acupuncture
Acupuncture has been in practice for over 3000 years with roots in Traditional Chinese Medicine. Other Asian countries have adopted the practice with their own variations. Accidents and Personal Injuries
Many people are unclear on what they should do if they've been in a car accident or had an accident in their workplace. The first step is to contact your insurance (for a car accident) or your HR department (workplace accident). Our front desk staff can help to answer your immediate questions via phone or email.
